Psychedelic Therapy in Boston

Last reviewed · June 23, 2026

Boston does not have legal psilocybin services. Massachusetts voters rejected Question 4 in 2024, so there is no regulated program, and Boston has not decriminalized psilocybin. Legal routes are ketamine and esketamine, clinical trials, or traveling to Oregon or Colorado.

The landscape

Psychedelic care in Boston.

Massachusetts voters rejected Question 4 in November 2024 by 57 to 43, so the proposed system of licensed psychedelic therapy centers and home cultivation never took effect. Psilocybin remains a Schedule I controlled substance under state law, and the same ballot question cannot return until at least 2028. In March 2026, state lawmakers advanced a pair of bills to create limited psychedelic-therapy pilot programs, but pilots and proposals are not the same as legal access today.

Boston itself has not passed a deprioritization measure, though neighboring Cambridge and several other Massachusetts cities have. Boston is, however, one of the country's leading centers for psychedelic research, with programs at Massachusetts General Hospital and Dana-Farber. Most people here access care through medical ketamine and esketamine, through clinical trials, or by traveling to licensed psilocybin services in Oregon or Colorado. Our concierge helps you compare these routes, review medical and psychological safety, and plan preparation and integration so you are matched with a vetted guide.

Costs & logistics

What to budget for.

Ketamine and esketamine: cost depends on format, number of sessions, supervision, and integration. In Boston, IV ketamine commonly runs several hundred dollars per session, with a starting course of multiple sessions. Esketamine (Spravato) is often insurance-covered for treatment-resistant depression because it is FDA-approved; IV and IM ketamine for mental health are usually off-label.

Psilocybin services in Oregon or Colorado: licensed centers set their own session pricing, usually including preparation and integration. Add travel and lodging. Verify licensing on the state program sites before booking.

Our concierge fees are separate from any provider's charges. Ask about HSA or FSA eligibility where applicable; policies vary.

Safety & screening

Before any session, cover this.

  • Share all medications, especially SSRIs and SNRIs, MAOIs, stimulants, and blood-pressure drugs.
  • Review cardiac history, seizure risk, bipolar spectrum, psychosis risk, pregnancy, and sleep apnea.
  • Confirm monitoring, chaperone policies, and emergency planning with any provider.
  • If considering at-home or compounded ketamine, discuss supervision and current FDA alerts with your prescriber.

Cultural & legal context

A closer look at Boston.

A failed ballot, an ongoing debate

Question 4 lost in 2024, and it cannot return to the ballot until 2028. Lawmakers have since shown more interest in narrow therapeutic pilots than in broad access. None has become law yet.

Not decriminalized in Boston

Seven Massachusetts cities have deprioritized entheogen enforcement, including neighboring Cambridge. Boston has not. State law applies, and psilocybin remains illegal in the city.

A national research hub

Boston hosts leading psychedelic research at Massachusetts General Hospital and Dana-Farber. That means clinical trials are more available here than in most cities, alongside a deep ketamine and esketamine market.

Care today: what people actually do

Most seekers pursue ketamine or esketamine with screening, join a clinical trial, or travel to Oregon or Colorado for licensed psilocybin services. Underground work also exists and carries risk. We do not broker illegal activity.
